Aged Care Nursing Adelaide – Quality Care at Home

Oracle Ability Services provides aged care nursing across Adelaide, helping older adults manage health conditions, recover from illness, and remain independent at home for longer. Our Registered Nurses deliver clinical care funded through Support at Home, DVA, NDIS, or arranged privately, whichever pathway fits your situation.

What Is Aged Care Nursing

What Is Aged Care Nursing?

It’s clinical nursing care specifically focused on the health needs of older adults, delivered by Registered Nurses rather than general support staff. It covers everything from wound care and medication management through to chronic disease monitoring and palliative support, always coordinated with the person’s GP and, where relevant, their broader aged care team. Nursing aged care differs from personal or domestic support in one key way: it involves clinical tasks that legally require a qualified, AHPRA-registered nurse.

Aged Care Nursing Services We Provide

Our nursing services cover the clinical needs that come with ageing, each delivered as part of a coordinated care plan:

Medication Management for Aged Care

Medication Management

Administration, organisation, and monitoring of medications, reducing the risk of errors that become more common as medication regimens grow more complex with age.

Wound Care for Aged Care

Wound Care

Assessment, dressing, and monitoring of wounds, including the slower healing and higher complication risk that often comes with older age. Read more on our Wound Care Management page.

Chronic Disease Management in Aged Care

Chronic Disease Management

Ongoing monitoring for conditions like diabetes, heart disease, or COPD, coordinated with your GP's chronic disease management plan. See our dedicated Chronic Disease Management Plan page.

Continence Care for Older Adults

Continence Care

Assessment, product recommendations, and catheter care delivered with dignity and discretion.

Personal Care for Older Adults

Personal Care

Showering, dressing, and mobility support, often delivered alongside clinical nursing as part of one coordinated plan. See our Personal Care Support page.

Palliative Care for Older Adults

Palliative Care

Comfort-focused nursing for older adults nearing the end of life, delivered at home wherever possible. See our Palliative Care Nursing page.

Meet Your Aged Care Nurse

Registered Nurses and Nurse Practitioners in Aged Care

Every aged care nurse on our team holds current AHPRA registration, and some clients with more complex needs benefit from the involvement of a nurse practitioner aged care services can draw on for prescribing and advanced clinical decisions. Whether your care is delivered by a registered nurse aged care visits are led by, or coordinated with a nurse practitioner, every plan stays connected to your GP’s ongoing oversight.

Meet Your Aged Care Nurse
In-Home Nursing Care for Older Australians

In-Home Nursing Care for Older Australians

In home nursing care Australia-wide generally follows a similar model: a Registered Nurse visits the client at home rather than the client attending a clinic, delivering care through a government-funded pathway, DVA or NDIS funding, or a private arrangement. An in-home aged care nurse typically supports wound care, medication, chronic disease monitoring, and general health assessments, adjusting frequency as needs change.

Oracle Ability Services provides this care specifically across Adelaide and surrounding South Australian regions. If you’re located outside this area, My Aged Care’s provider search can help you find an in-home nurse for aged care closer to you.

How Aged Care Nursing Is Funded in Australia

This can be funded through several different pathways, depending on your circumstances:

Support at Home for Older Adults

Support at Home

Support at Home is the Australian Government's current in-home aged care program, which replaced Home Care Packages on 1 November 2025. It's accessed through a My Aged Care assessment and funds care across 8 classification levels, released as quarterly budgets. Clinical care, including nursing, is fully government-funded with no means test. From 1 October 2026, personal care services will also become fully government-funded with no co-contribution required.

DVA and NDIS Funding for Aged Care Support

DVA and NDIS Funding

Eligible veterans can access this kind of support through DVA Community Nursing, while NDIS participants may access nursing connected to a disability-related health need through their NDIS plan.

Private Aged Care Nursing Services

Private Aged Care Nursing

This option is available for anyone not yet approved for government funding, waiting on a My Aged Care assessment, or wanting support beyond what their current funding covers. No referral or waitlist is required to arrange care this way.

Supporting Older Adults at Home

Supporting Older Adults at Home

Nursing care for older adults often makes the difference between ageing at home comfortably and needing to move into residential care earlier than necessary. Aged care at home works best when clinical monitoring catches small changes before they become emergencies, medications are managed correctly, and a consistent nurse understands a person’s history well enough to notice when something’s off. Aged care support services that combine nursing with personal care, rather than treating them as separate services, tend to work best for exactly this reason.

Aged Care Support Delivered With Genuine Continuity

Our service

Delivered with Genuine Continuity

At Oracle Ability Services, we believe ageing well at home depends on consistency, the same nurse, the same understanding of your history, visit after visit. Our nursing team works closely with GPs and, where relevant, My Aged Care providers, so nothing falls through the cracks between your funded supports and your clinical care.

Why we better

What Makes Our Nursing Support Different

Plans

Registered Nurses, Not Generalist Carers

Every clinical visit is delivered by a qualified, AHPRA-registered aged care nurse, never a support worker standing in for clinical tasks.

Registered Nurses

Consistent Faces, Not a Rotating Roster

The same nurse attends most visits wherever possible, so you're never re-explaining your history to someone new.

Clinical Oversight

Coordinated with Your GP and Aged Care Team

We communicate directly with your GP and any other providers involved, so your care stays joined up rather than fragmented.

Seamless Coordination

Care That Adjusts as Needs Change

Ageing needs shift over time. Your nursing plan is reviewed regularly, not set once and left alone.
