Community Nursing Services to DVA Card Holders
Our core service supports Australian veterans by delivering community nursing care in their own home. The aim is to enhance independence and health outcomes, and to avoid early admission to hospital or residential care.
We provide low-risk, primary care nursing using simple clinical interventions that are tailored to your specific health needs and nursing care plan.

Who is eligible
- Holders of a DVA Veteran Gold Card (all assessed nursing and personal care needs covered)
- Holders of a DVA Veteran White Card (care related to accepted service-connected conditions)
How to get started
A written referral from your GP, nurse practitioner, hospital treating doctor, or hospital discharge planner is required. Contact us or have your referrer send the referral directly to our team.

Home and Community Needs Assessment
Before care begins, a Registered Nurse will visit your home to conduct a comprehensive, personalised assessment. This ensures all services are appropriate, evidence-based, and responsive to your current health situation.
The assessment covers
- Current health status and clinical needs
- Medication review and administration requirements
- Personal care needs
- Risk factors in the home environment
- Short- and long-term care goals (developed in collaboration with you)
- Involvement of family, carers, and other health professionals

Highly Specialised and Complex Care
For clients with more complex or high-acuity nursing needs, our experienced nurses provide highly specialised care, including management of chronic conditions, post-acute recovery, and palliative care.
- Palliative and end-of-life care support
- Complex wound management
- Advanced clinical symptom monitoring
- Post-surgical or post-hospitalisation care
- Coordination with specialist medical teams

Medication Management and Administration
Safe medication management is critical to health outcomes. Our Registered Nurses conduct a comprehensive medication review upon admission and provide ongoing medication support throughout your care.
Includes
- Medication review at commencement of care
- Safe storage guidance
- Proper administration (in line with a signed medication chart or authority)
- Monitoring for side effects and interactions
- Liaison with GPs and pharmacists as needed

Wound Care Management
Our nurses specialise in the assessment and management of chronic and complex wounds, following evidence-based treatment protocols and developing personalised wound care plans.
Includes
- Initial wound assessment and classification
- Formulation and ongoing review of wound care plans
- Application of appropriate dressings and treatments
- Monitoring for infection and healing progress
- Documentation and reporting to treating practitioners

Coordination of Services
Navigating health services can be complex. Our community nursing staff acts as your care coordinator — connecting you with the right services, professionals, and resources to support your overall wellbeing.

- Referral to GPs, specialists, and allied health professionals
- Communication with hospitals and discharge planners
- Coordination of DVA-funded services
- Care plan reviews aligned with your evolving needs
- Liaison with family members and carers (with your consent)
