How to Start Community Nursing Services with Oracle Ability
Getting started is simpler than most people expect. Here is exactly what happens:
- Get a referral. Ask your GP, nurse practitioner, treating hospital doctor, or discharge planner for a written referral. Not sure how? Call us and we will guide you through it.
- We assess. A registered nurse visits your home for a comprehensive needs assessment, at a time that suits you.
- We plan together. You receive a personalised care plan built around your health goals, preferences, and routine. Nothing is decided without you.
- Care begins. Your community nurse home visits start, with regular reviews to keep your care matched to your changing needs.
Good to know
A new referral is valid for 12 months. If you have been out of care for more than 28 days, or you want to switch from another provider, we can arrange a fresh referral quickly and without fuss.
