A new referral is valid for 12 months. If you have been out of care for more than 28 days, or you want to switch from another provider, we can arrange a fresh referral quickly and without fuss.

Yes, if you hold a Veteran Gold Card (all assessed nursing needs) or White Card (service-connected conditions). Call the DVA Provider Enquiry Line on 1800 550 457 to confirm.
Get a written referral from your GP, nurse practitioner, or hospital. We then complete an in-home nursing assessment and build your personalised care plan with you.
All care is delivered face-to-face in your own home. Where appropriate, remote support by phone or online can also be arranged.
Yes. Every client receives an emergency contact number available 24 hours a day, 7 days a week, every day of the year.
