We work in close collaboration with your GP, treating specialists, hospital teams, allied health professionals, family members, and community services — so that every element of your care is aligned and nothing is missed.

- Referral to GPs, medical specialists, and allied health professionals as needed
- Direct communication with hospitals, discharge planners, and rehabilitation teams
- Coordination of all DVA-funded services to ensure a seamless care experience
- Regular care plan reviews to keep services aligned with your evolving health and personal needs
- Liaison with your family members and carers — with your consent — to support a shared understanding of your care
- Support with navigating DVA entitlements and understanding what services are available to you
- Escalation of concerns or changes in health status to your treating practitioner promptly
Who is eligible
- DVA Veteran Gold Card holders — coordination of services included as part of assessed community nursing care
- DVA Veteran White Card holders — coordination related to accepted service- connected conditions
How to get started
A referral from your GP, nurse practitioner, or hospital discharge planner is required to commence community nursing services, which includes coordination support. Once your care begins, your nurse will take on the coordination role as an integrated part of your ongoing care.

Nursing services start in line with your agreed care plan. Your nurse will document every visit, and your plan is reviewed regularly to ensure it continues to meet your evolving needs.
