DVA Community Nursing Services

Oracle Ability Services delivers professional community nursing care directly to your home. Our core service supports Australian veterans and their families — helping you maintain your independence, recover more effectively, and avoid unnecessary hospitalisation or early entry into residential care.

We provide low-risk, primary care nursing using evidence-based clinical interventions, all tailored to your individual health needs and nursing care plan.

Who is eligible

  • Holders of a DVA Veteran Gold Card — covers all assessed nursing and personal care needs
  • Holders of a DVA Veteran White Card — covers care related to accepted service- connected conditions

How to get started
A written referral from your GP, nurse practitioner, hospital treating doctor, or hospital discharge planner is required. Contact us or have your referrer send the referral directly to our team. Referrals from a GP or nurse practitioner are valid for 12 months.

Our team makes the transition simple — we work with your GP or treating practitioner to obtain the necessary referral and assessment, then build a care plan around your specific health needs.

A new referral from your GP or nurse practitioner is valid for 12 months. If you have been out of care for more than 28 days, or wish to change providers, contact us today — we will guide you through every step.
