Home and Community Needs Assessment
Before any nursing services begin, your Oracle Ability Services Registered Nurse visits you at home to conduct a thorough, personalised assessment. This is a critical first step — it ensures every service we provide is appropriate, evidence-based, and truly responsive to your current health situation and personal circumstances.
Your assessment is conducted with you at the centre. We listen, we ask questions, and we make sure your goals and preferences guide everything that follows.
What's Included in Your Assessment
- Review of your current health status and clinical nursing needs
- Comprehensive medication review — including what you take, how you take it, and any concerns
- Personal care needs — showering, dressing, mobility, and daily living activities
- Risk assessment of your home environment — identifying hazards and safety considerations
- Identification of short- and long-term care goals, developed in collaboration with you
- Review of existing support networks — family, carers, and other health professionals involved in your care
- Planning for referral to additional services if required
Who is eligible
- DVA Veteran Gold Card holders — assessment covers all identified nursing and personal care needs
- DVA Veteran White Card holders — assessment covers needs related to accepted service-connected conditions
How to get started
Your GP, nurse practitioner, hospital treating doctor, or discharge planner will provide a written referral. Once received, we will arrange your in-home assessment at a time that suits you.
